Your receipt is on its way by email. The next step is to tell us about your family. The more you can give us here, the less time we spend on groundwork and the more on the records themselves.

Do not worry about gaps. Almost every client starts with incomplete and partly incorrect information. Spelling variants, family stories and uncertain dates are all useful - include them and mark them as uncertain.

Helps us match your details to your payment. It is on the receipt Stripe emailed you.
Include first names, surnames and variants: Wojciech / Albert / Adalbert, Kowalczyk / Kowalczik / Kovalchick.
Copy it exactly as it appears in the document, mistakes included. A wrong spelling is still a clue.
Spouse, parents, siblings, children - with any dates or places you know, however uncertain.
Naturalization papers, ship manifests, church records, obituaries, letters, photographs with captions.
Up to 5 files, JPG, PNG or PDF, 10 MB each. Phone photos are fine as long as the text is readable.

What happens next

  • Within two business days, we review what you have sent and confirm the research plan by email
  • We begin work in the archives and collections relevant to your case
  • You receive your written report with images, translations and sources

You will hear from us within two business days of sending this form, with the confirmed research plan and the expected delivery date. If anything is missing, we will ask.
